Enter a User ID and password to gain access to the UnForm archive browser interface.
User ID's and passwords are case-sensitive, meaning they are sensitive to upper and lower case letters.
You may need to toggle your Caps-Lock key if your login attempt fails.
Multiple language files are configurable by the administrator; if more than one language is available it will be presented in the drop-down list.
The Assist button can be used to request login information if you know your User ID or have an email address on file associated with an UnForm User record.
Clicking on the Assist button presents a form underneath the login prompt which asks for either a User ID or email address.
Enter a User ID or a valid email address to request password information.
If valid information is provided, appropriate login inforamtion is emailed to the address on file.
